CMT. No GRE/sGRE. 1 preprint, 3 years of CMX research, 6 months of CMT research. No interview. GPA: 4.05/4.3 = 3.77/4.0 The admissions committee was very impressed with your application and had put you on top of our waitlist. It turns out that we might have one opening and so we wanted to check with you to see if you had any interest in our program at this point?" I guess this count as an admission? Turned down immediately after receiving this email for another offer.
